Update express-server example to latest version

In an attempt to embed horizon in an express app, I realized that the example in this repo is for horizon 1.0.1 and doesn't work out of the box with current horizon version.

I updated the necessary files, and created a README.md that shows a tutorial to facilitate the process for new comers.
