Start Date and End Date in the same Form

Hi,

I have this addon installed on a website of mine. I need a form that will include a Start Date and an End Date. The Start Date works fine and lets me select my options but the End Date does not display anything.

How can I make it work with 2 datepickers in the same form? Page: https://mirtrans-express.com/inchiriaza/
