Rename extension to OpenShift Toolkit

With the release of 1.0.0 version of the extension it will be rebranded as OpenShift Toolkit. We need to update the Readme, gifs, reference docs and Jenkins.

The package.json should be updated to reflected the changes in VSCode Marketplace.
