Installer fields must be grouped and reordened

Some installer fields must be grouped for easy filling. Application specific fields must be on it's own group while administrator specific fields must be grouped like that. Also, these groups must be ordened in a way to make it fast and funny to fill. I suggest three groups:

• [ ] Application settings: In this group Choko will provide fields for the application name and email address.
• [ ] Administrator settings: In this group Choko will provide fields for the administrator username, email address and password.
• [ ] Advanced settings: In this group Choko will provide fields for the database server, port and name, and any other advanced setting that can be interesting to the user installing Choko.
