Generic structure for "a directory of commands with help and plug-ins"

We have:

• commands
• patterns
• elements
• outputs
• settings

All of which are similar, all of which need help, all of which would like to accept plug-ins.

(Settings are a trickier case and probably will have to be done separately.)
