Support item name mapping

Support defining custom maps from attribute name to item name, e.g. to support item names with dashes, leading digits, etc.

Based on Commit #31 (included it's included commits) and #32.

When merging an #28 to #31 and #32 without creating a merge commit (using github's rebase and apply method), this will apply cleanly.
