Remove Implementations from Extensions / link to STAC Index

In order to accurately assign maturity levels to all the Extensions, we need to have Implementations added for each one in the README of each extension.

Changed this to beta2, to give implementations a bit more time to come online. I'd propose we should count everything from 0.9.0 onwards, since we had the major reorganization at that version. Though I could be ok to count some older catalogs...

Discussed on call. We don't want to have to maintain this list in the repo, but agreed it is useful to see implementations. Decided we will link to stacindex.org, which will implement pages that list all the catalogs that use a particular extension.

The extension pages is probably not available for RC1, but I can at least (manually?) give an indication one extension use so that we can evaluate the maturity levels.

Cool, real 'pages' definitely aren't needed. Can you just paste the list in here? Most important is how many different catalogs use each extension.
