Use both the speaker and the headphones
Case scenario: Sometime I forget my headphones connected to the smartphone when I'm not using them, this often happens when I go to sleep. I don't hear the alarm when this happens so I don't wake up. This can be prevented by adding a setting: Use the speaker if headphones are connected
I would also like this, for when I'm playing music from my phone and can't hear the alarm (because my phone, and the speakers, are in the other room; I can hear the music lower but not the notification sound).
Can I suggest simply playing through both channels, instead of adding a setting, and only add the setting if someone complains about it?

Great, I was thinking of that too.
I almost always prefer features be turned into settings when you have anything that might be used by two camps of people.
One of the biggest mistakes I see software companies make (different situation, I know)-- is where they change a feature to suit the people who've been complaining, without ever realizing they weren't hearing from the 0-99% of the people who were perfectly happy with the way that it was.

In my experience with open source software, the opposite is more common: adding settings in anticipation that there will be two camps, and ending up with a gigantic, impenetrable mess of settings that are useless to anyone who doesn't want to spend hours looking through them all.
I think adding an additional setting can make sense, but only after making sure that the new way isn't just straight-up better for everyone.
Hence, I propose(d) making the change and waiting to see if antibody actually has a problem with the new behavior, before adding another setting.
