Add stim.Circuit.diagram

• Change TICK convention so that first TICK is index 1 instead of index 0
• Fix circuit timeline svg diagrams using width/height instead of viewbox property
• Add stim._DiagramHelper with HTML viewers for text and SVG
• Add a bit of whitespace padding around svg timeline diagrams
