code-of-conduct: duplicates removed: report guide

Removed duplicate content, adding a clear reporting guide, and hopefully a better flow.

@sharmi Can we have a separate page for the reporting? That will keep the CoC short, so that people end up reading it.

Instead we can just have section, indicating who this applies to. And there we can mention that this applies to speakers, exhibitors and participants.

And anything specific to them can be indicated there as well.

I would prefer if I can use a different id and phone number and redirect them to my id. Hopefully we can reuse the same next year also.

Can we have a [email protected] and forward it to all members of the committee?
