Authors not being added to "Notify" box

The author of content should be automatically added to the "Notify" box but that's not happening currently:

Related issue:

The 'Notify' function seems to be not working properly. For example, if I click the 'Stop notifying me' link on the posts list page nothing happens. If I edit the post and click the button to remove my name from the notify box, then update the post, my name reappears in the notify box.
