Extend test meta box form with extra fields (shipping/billing address, lines, etc.)

Consider adding separate address fields for shipping and billing:

These extra field could be optional, we could add a checkbox to reveal these extra fields. If WooCommerce is active we can also autofill some of these fields from the user profile settings.
