Rename RemoveAnswer to RemoveDocumentAnswer

Answer is not a entity which exists as itself like Question but is always linked to a document.

That's why mutation to save document answers are called saveDocumentAnswer so for consistency removeAnswer needs to be renamed to removeDocumentAnswer.
