[Draft] Form component APIs

While working on Autocomplete design updates, I began drafting updated CSS for general form components (which Autocomplete depends on). I decided to build the markup in PVC instead of drafting in PCSS, though I'm not sure we will even move forward with PVC form components. In any case, if we do move forward at least we have a bit of a start.
