readme: Readability and convenience improvements

It mainly adds the following:

• A Usage section. I had difficulties using the extension and couldn't find it documented anywhere. I ended up looking in the source code to find out how to use this.
• Fix the Test in (Chrome|Firefox) locally sections to:
  • Not reference non-existence npm scripts
  • Be headings that actually describe its content
