I also have this problem. I have an LVM install that I have DD'ed onto an rcraid array. mounting everything, chrooting in installing the driver, and generating the FSTAB works...
Upon further investigation the issue appears to be rcraid + encryption. After adding rcraid and regenerating the initramfs, it never bothers to ask for a decryption password. So it goes...
