Add references defaults

Preliminary work towards resolving https://github.com/pow-auth/pow_assent/issues/168

First step of supporting references defaults in the migration is done, but I'll have to figure out how to infer the references defaults from the extension assocs. It isn't entirely obvious how this should work.
