Missing changelog entries?

I just noticed that recent releases are missing changelog entries since v4.0.0-beta.5. Also noticed that this is at 4.1.2 on npm, although there hasn't been a release on GitHub since 4.0.0.

Are there major breaking changes between 3.x and 4.x? Anything to consider when migrating?
