It will be the natural earth place shapefile, which is only used on low zooms.
That file is not purely ASCII. As distributed, the Natural Earth shapefile has an encoding error. To fix this, the osm-carto download script runs ogr2ogr on it, fixing it up....

Setting `shield-allow-overlap: true;` so I can see everything it **attempts** to place gives me , with the Abbotsford dot entirely in the top MT and overlapping with the Vancouver text

Ah - if you're using a system node-mapnik instead of a locally installed one, then it'd be definitely using a system mapnik of some form.
