Not a bug with cartographer itself but with the game. Maybe it can be looked into in the near future.
@NukeULater is this not what some of your changes attempted to address? Also, I'm not sure I like the idea of putting normal game issues in with our repo as our changes aren't what cause this and it's unrelated to our code.
This isnt anything new .Existed since long back and usually tends to happen due to meleeing side ways or lags

That's why I feel like this is a won't fix on our end, meaning it's basically spam in the repo issues list.
Does this still occur in MCC?
Not sure about MCC, perhaps it was fixed. It happens because of the 30hz->60hz update in H2PC.
