Good afternoon colleagues. Please tell me how to make sure that the user sees the assigned specialist and the time until the application is resolved. After installing the plugin, this information disappeared

switched the Self-Service user profile to normal mode and these fields appeared for the user. Now the problem is that by default the standard application template opens and not the form-creator. How to make form-creator questions open in your Self-Service profile
