How about the following syntax:

dicomsort src_folderroot tgt_folderroot/%tag1_%tag2/%tag3/%tag4_%tag5

So, for example:

dicomsort data sorted/%PatientName/%StudyDate/%SeriesDescription-%InstanceUID.dcm

would create folders like:

sorted/JohnDoe/2013-40-18/FLAIR-2.dcm
