I think this is because NextDNS is deduping A/AAAA/HTTPS queries for a domain. Happens on Apple devices for example. See here: https://help.nextdns.io/t/m1hfhd5?r=y4hfh78
I think I found a reason for this. It is AS3320 - DTAG Networtk - maybe this is also the issue with problems over here https://github.com/AdguardTeam/AdGuardDNS/issues/777

I did adopt the smokeping testing to TCPPing (5 pings every 300 seconds). (https://oss.oetiker.ch/smokeping/probe/TCPPing.en.html) These are the results for the last 12h. 
Again those spikes during primetime....

Locking the screen again and unlocking it after did resolve the issue. Strange behavior
