The LED fade is tied to the update cycle, so if you reduce the simulation speed, they will fade faster (in simulator time). Obviously this is unrealistic.

It takes 2 for me. I will probably need to make it configurable to make everyone happy. I see Firefox on Mac takes many more scroll steps to switch.

The top one that says "out" is not a labelled node, but an analog output. Outputs and Labels->Analog Output. It doesn't do anything. You can optionally view its voltage instead...
So in the 3rd one, the LED is forward biased, and the diode is reversed? It seems right. The saturation current of the diode (roughly, its reverse-bias current) is 18nA....
