`prepub` toggle no longer included?

This is a continuation of #195

In that thread you mention a prepub toggle. The document that needed a watermark back then is now ready for prepublication, but I couldn't find the prepub toggle in either the csasdown setup from that document, or the most recent test res doc I generated with the latest csasdown install.

An issue search for "header" or "prepub" didn't turn up anything.

When I added "Not for further distribution" in the header toggle of index.Rmd, it shows up on the colophon page, but not on subsequent pages. The same thing happens with the default "Draft working paper --- Do not cite or circulate" header in the test res doc generated with the latest csasdown install.

CSAS needs the "Not for further distribution" header on every page, I think.

Am I missing something basic?
