add option to set custom header and footer during export of the table

Details

When you export table using option "Export the table" to pdf there is no option to add custom header or footer.

It would be nice to be able to either: enter text for the header, and footer in the configure screen for "export the table" to pdf, or to select custom text files for header and footer.

Posting here because the idea seems closely related : when exporting the result of a SQL request (or a view), an option to indicate what was the request (or the definition of the view) would be a great addition : sometimes I have a PDF report from some time ago, and I do not remember what were the exact parameters of my request.
