add option for per-client/per-connection bandwidth limit

As title, can limit bandwidth for each connection, and add a -raw mode if someone just want to limit bandwidth yet not to use any HAProxy in case. ps. I use it for my minecraft server to prevent some player using up all my upload bandwidth if they kept teleport/flying/fast-moving.
