Update documentation for creating tokens

The HipChat API and its documentation has changed somewhat, making it hard for users to create the required token and scopes.

You need to select multiple scopes to have access 2016-02-08 18 38 30

+1 on getting this on the homepage. I spent almost an hour trying to figure out what I was doing wrong until I found this ticket.

+1 I gave up trying for a while and wrote a webhook version but thankfully i found this the second time around.... add that screenshot to your readme.
