rewrite slack_notification to post blocks

Slack changed their incoming webhook API to use blocks instead of attachments in 2019. I rewrote the slack_notification QuickSilver script to use the current API. I also fixed the link to the environment's dashboard, and generally cleaned up the code.

This repo belongs to the cms-ecosystem squad. We are aware, but we have a large backlog. Not sure when we will get to reviewing the PRs in this repo.
