Subsequent Modals always have buttons disabled

Hi, I think this might related to the fix done for #28. Now, the 2nd modal onwards has its buttons disabled by default

I have removeOnHide: true so the element itself is destroyed.

@jasonnathan, I got tired of waiting for him to merge my pull request and have created a new atmosphere package based on my fork of this repository. I think you will find it fixes your problems. https://atmospherejs.com/jchristman/reactive-modal
