fix issue #14 : don't work with WikiLinks have both block reference `#` and alt texts `|`

You can now convert [[WikiLinks]] with #, ^ and | correctly.

• After: [Alt](File#Block)

A paragraph reference one:

• Before: [[File#^1f1e33|Alt]] goes to [](File#^1f1e33%7CAlt)`
• After: [Alt](File#^1f1e33)
