Move placebo adherence

  1. placebo recipes:
    1. move most to ice-ought-private and refactor in this PR: https://github.com/oughtinc/ice-ought-private/pull/20 2. move recipes that are consumed by PlaceboTree and only by PlaceboTree
    2. just delete some recipes that I don't think we'll use anymore
  2. adherence recipes: delete now. I didn't want to bother moving them, but if we want to use them in the future we should consider refactoring them first to match the placebo recipes
