Remove bootstrap-toc

bootstrap-toc doesn't work properly when files are opened locally on a machine, and the project is not getting many updates. It also needs us to include jQuery.

We can get basically the same functionality by manually using a list group and the already-enabled scrollspy.

This should work wherever the files are deployed.

As part of this also need to check the scrollspy offset - seems to be moving the toc too quickly.
