Using clca initialize:

openssl.cnf

0.domainComponent = Top
1.domainComponent = Middle
commonName = Root CA 1

Creates this certificate

Issuer: DC=Top, DC=Middle CN=Root CA 1
Subject: CN=Root CA 1, DC=Top, DC=Middle
