[feature] Add notes and radius groups to batch user creation

Some feedback coming from users of OpenWISP RADIUS regarding batch user creation:

  • add a note field (textarea as we have in other models like Device in OpenWISP Controller)
  • allow to specify the radius user group which the users will be assigned
