Seems like this topic has gone a bit stale, but it is definitely worth clarifying this point as it can be common in some non-RPC contexts.

This has been "hinted" at in https://github.com/opentracing/specification/issues/52#issuecomment-284235106 where the "join" case could add references from these "distant" spans (from different traces)
