Remaining issues for image annotation

• [ ] Handle secondary focus in a reasonable way
• [ ] Introduce a useful notion of resource position, corresponding to PDF pages and audio/video timestamps. Currently these are just "zero" since image annotations are media fragments without a temporal component. It'd be nice to do something more useful with these.
