source tag should be added to changeset, and no more to single objects

According to current screen layout:

The "S" button adds a tag with source=... to a selected object.

This practice is outdated, because a source tag is to be added better to the whole changeset itself. Have a look at JOSM how it is solved there in upload dialog, or read http://wiki.openstreetmap.org/wiki/Key:source for more details.

So please

1. remove that S button, and
2. add a field for a source tag to Merkaartors upload dialog.
