Edit/Remove Notifications for a Monitor

Description

Currently when we add notifications issues are

• not able to delete already active ones
• not able to edit active ones

Also if possible to segregate notification integration by their provider type and to make it more visual appealing this'll be an issue when more providers are added

What do you mean by 'active ones'?

I agree, it is missing some more details/table columns. It is very blank currently as we don't have much more to show.
