fix other in the template tags

other at the moment has funny behvaour, e.g. in location, we have empty and ---- as another option, then if you're on Other and enter something in the row above goes empty

My proposed fix would be to just delete it - I don't think it's used outside of UCH and I don't think we were ever happy with the UX?
