Move "Note" field below "Direct links"

@SaraSchnadt pointed out that in her proofing, some elections have multiple relavant direct links and that it's nice to be able to describe them. Because of the way we parse "Direct links", we can't put notes or comments in that textarea. So, we should move the "Note" field out of the "Notes" fieldset and put it below the "Direct links" field so it's quick to describe what's up when there are multiple URLs in the direct links field.
