Attach file when grading submissions - attach feedback

Hi,

We are currently looking some way to integrate the file input to attach any file (Doc, PDF, Image) to the given score and comment for any ORA submission, as shown in this image:

We want that the teacher choose the rubrics... good, poor, and so on, then put the comment of the response, attach a file with the feedback of the activity (If the student uploaded some file or doc), and then upload the file, that file will be visible to the teacher and student for download.

Hope to be clear.

Has someone any idea of how to implement this?
