onie-fwpkg - duplicate update names collide

The recording keeping mechanism uses the filename of the update as an index into the list of update records. That causes problems when installing the same update twice. The most recent update record clobbers the previous record.

Note however, that onie-fwpkg show-log does display the complete history.

This problem is pretty minor.
