Structured annotation templates
Instructors should be able to define a template of how an annotation should be structured. Upon creation of the template instructors should be able to define if students can or must fill in the template when creating a new annotation. Within the template the instructors should be able to define which types of content the students have to fill in, as well as in which order. This requires that annotations can consist of more then one type of content, see issue #370.
While defining the template the instructors should be able to limit which categories are visible to the students as well as which categories are visible.
As a user: How am I able to use such a «multi content annotation template» (which seems to me a better fitting name)? Do I click on the new button to create a multi content annotation and am I then asked if I want to chose one of the existing templates or not?

This should be up to the instructors. Instructors should be able to mark templates as optional or mandatory. When a template is mandatory the user will only be able to use this template when creating multi content annotations, when it is optional the users should be asked if they want to choose an existing template.
