Add manager doesn't add fields - SNMP page

Where: On the SNMP page

Current / Description: After the info for one manager has been entered and 'Add manager' is clicked, the hostname and port fields get collapsed into the list view and no new empty fields appear. The user then has to click on 'Add manager' again to get new fields.

Fix to: Upon clicking 'Add manager' collapses filled in fields that do not have errors and more importantly provide a new set of fields for the additional manager to be added.
