Support for surveys / Questionnaires

Details : Requirement Elicitation can be done through app using 2 ways, Surveys and Feedback (Needs Community Approval).

For Survey, we can add Multiple Choice Questions, because user wouldn’t want to like descriptive answers to 5 -10 questions. I would also like to add a completely new functionality into Survey, like question which ask user to rate their experience (0 – Poor, 5 – Excellent ). For instance, if a user rate 0 for any question, then show a EditText and ask user suggestion on how to improve that issue, as shown below.

For Feedback, user can give descriptive review from his/her experience and even he/she can give suggestions on what new functionalities, does he/she wants, and on how to improve existing functionalities.
