Debugger attach to file

Today, the debugger can launch an eval session that creates and evaluates a query, internally attaching to it through a debug tracer.

As part of query result output, OPA can attach a trace log (e.g. --explain in opa eval; explain query param in server REST API). The debugger should have the ability to attach to this pre-evaluated trace (read from file or pipe). This would benefit in troubleshooting remote query evaluations that aren't easily replicated locally, but where it is possible enable tracing on the remote service.
