Unable to create a new Examination

Problem

Unable to create a new Examination. The Add examination form always results in failure.

Steps to reproduce

  1. Go to myHealth in the bottom bar.
  2. Press Add Examination
  3. Try filling out the form and submitting it

I think this might be an issue with the vi and dev servers being down. I was able to create a examination before today.
