Add pull request template

As with #40, we want to make it easy for contributors to provide useful descriptions of pull requests.

I have a branch with this on, but as some of the items on the checklist refer to things we haven't implemented yet (e.g. pytest, pre-commit) I'll hold off on making a PR for now.
