Review the new portal submission issue template

We're getting frequent empty submissions of portals to be added. The issue template should be changed in order to:

  • [ ] include a notice to contributors that they must fill in all of the required fields
  • [ ] make the template fields conform to the currently adopted data schema
  • [ ] include a @ notification to maintainers
